How to Apply for MassHealth and Commonwealth Care

STEP 1. APPLY TO MASSHEALTH

Before you can choose a BMC HealthNet MassHealth or Commonwealth Care plan, you must apply to MassHealth. MassHealth determines which state program you are eligible for.  

You must complete a Medical Benefit Request form. If you need help applying, call us at 800-792-4355 or e-mail us at helpmeapply@bmchp.org for FREE help applying. Please follow these guidelines when e-mailing us:

  • Your name*
  • Your phone number
  • Your e-mail address; AND
  • The best times and days to reach you
  • Any personal identification information such as your social security number
  • Information about medical conditions or treatments
  • Questions about medical conditions or treatments

GET HELP APPLYING IN PERSON

You can get free help applying at a number of places, such as a community health center, agency, or advocacy group or a hospital or MassHealth Enrollment Center. Many places can enter your information directly into MassHealth's online application system, also called Virtual Gateway. This may speed up the processing of your application. Find out where to apply in person.

KEEP IN MIND THAT WHEN YOU APPLY, YOU’LL NEED:

  • Two recent pay stubs or
  • Last year's U.S. tax return (if self-employed or have rental income) or
  • Social Security award letter
    AND
    • Other income (i.e., child support, unemployment, etc.)
    • Proof of citizenship/national status (copy both sides)
      • U.S. passport or
      • Certificate of Naturalization (DHS# N-550 or N-570) or
      • Certificate of U.S. Citizenship (DHS# N-560 or N-561) or
      • U.S. birth certificate and a Massachusetts ID or driver's license

STEP 2. ONCE YOU QUALIFY, CHOOSE BMC HEALTHNET PLAN

Once you qualify for MassHealth or Commonwealth Care, the state will send you an enrollment package that tells you how to select a health plan.

If you qualify for MassHealth, you must call 800-841-2900 to choose BMC HealthNet Plan as your health plan. If you do not select a MassHealth health plan within 14 days of the date on the enrollment package letter you receive from the state, the state will automatically assign you to a health plan.

If you qualify for Commonwealth Care, you must either call 877-623-6765 to choose BMC HealthNet Plan, or do so online at www.mahealthconnector.org.
